A note on confidentiality. Clinical information shared with your clinician is protected under state law and the Health Insurance Portability and Accountability Act (HIPAA). Those protections are described in our Notice of Privacy Practices, which you receive at intake. This website policy covers only the information collected through this website.
1 · Information collected
Information is collected only when you choose to provide it, plus basic technical data:
- Request for consultation form: your name, email address, phone number, and the message you enter under “How can we help you?” Please provide only general information about the services you are seeking. Do not include detailed clinical or medical history in the form.
- Technical data: like most websites, basic technical information such as IP address, browser type, and pages viewed may be processed by the hosting, analytics, and font providers listed below for security, delivery, and site statistics.
This website is not for emergencies. If you or a loved one is experiencing a life-threatening emergency, do not use the form. Call 911 or the 988 Suicide and Crisis Lifeline.
2 · How it is used
Your information is used solely to respond to your inquiry, schedule an initial consultation, and maintain the security of the site. It is never sold or rented, and it is not used for advertising.
3 · Third-party services
This site relies on a small number of providers, each governed by its own privacy policy:
- Google: website analytics (Google Analytics), spam protection on the form (reCAPTCHA), and web fonts.
- GoDaddy: website hosting and content delivery, including the Cloudflare network GoDaddy uses to serve and secure the site.
- Google Workspace: email delivery for form submissions and practice email.
4 · Cookies
This site uses Google Analytics to understand how visitors find and use the site. Google Analytics sets cookies that record anonymous usage data such as the pages viewed and the source that referred you. It does not collect your name or contact details. You can opt out with Google’s Analytics opt-out browser add-on or by blocking cookies in your browser.
Google reCAPTCHA may set a cookie where the consultation form appears, to tell people apart from automated bots. GoDaddy’s Cloudflare network may set short-lived security cookies that protect the site from automated abuse. Blocking these may prevent the form from working.
5 · Children’s privacy
We serve children and teens, but this website is intended for use by parents and adults. We do not knowingly collect information from anyone under 18 through this website. A parent or guardian should submit the consultation request on a minor’s behalf.
6 · Retention and your rights
Form submissions are kept only as long as needed to respond to you and maintain reasonable records. You may request access to, correction of, or deletion of information you submitted through this website by contacting us below. Clinical records are governed by separate legal retention requirements and the Notice of Privacy Practices.
7 · Links to other sites
This site links to outside resources for parents and clients. We do not control those sites and are not responsible for their content or privacy practices.
